The Daviess County Clerk’s Office is an approved Passport Acceptance Facility.  You must apply in person if at least one of the following statements is true:

• You are applying for your first U.S. passport
• You are under age 16
• Your previous U.S. passport was issued when you were under age 16
• Your previous U.S. passport was lost, stolen, or damaged
• Your previous U.S. passport was issued more than 15 years ago

What to bring

• Applicant must be present.  Minors (age 15 and younger) require BOTH parents to be present as well.  Those 16-17 years of age must have one parent present.
• Certified birth certificate from the state where you were born.  (Hospital birth certificates are not acceptable).
• Valid Kentucky driver’s license.  A second form of identification is required if presenting an out-of-state license. 
• Two separate payments will be collected:
  • US Department of State: This fee must be paid with a check or money order only.
  • Daviess Co Clerk: This fee may be paid with cash, check, money order, or credit/debit card (a vendor fee will be applied).
