- Type of application desired
- Vehicle Identification Section
- Odometer Disclosure
- Owner/Buyer Section, including current owner’s name, SSN, and address
- Check the appropriate boxes for the lien information
- Signature and Notary
Fees
$6.00 – Application notarized out of our office
$10.00 – Application notarized by the County Clerk’s office

A registration will be mailed from our office. The new title will be mailed to the registered owner within 3-4 business days. (If pending a lien, the title will not print for 30 days.)
